9-week palliative care virtual training program

 

About

    • Supporting the Journey Home is a virtual educational program for interprofessional community care teams working in First Nation communities.
    • The program will provide practical tips and strategies for incorporating an early palliative approach from a First Nations perspective. 

Program Commitment 

    • 9 weekly 1.5-hour sessions offered twice per week
    • Virtual learning (Zoom with audio and camera)
    • Participate in Circle Reflections
    • Complete weekly exercises
    • Access to an online learning platform (Moodle)

WHO IS THIS FOR

  • Community care providers who have a caregiving role (e.g. physicians, nurse practitioners, patient coordinators, PSWs, CHNs, CHRs, cultural workers, Elders, etc.).
  • Must be working within a First Nations community setting or coordinating care with community care providers based in a First Nations community in Ontario (e.g. external care providers coordinating/delivering care for First Nations people)
